Free new customer intake form (fillable PDF)

A form for taking down everything you need from a new customer on the first call or visit, from contact details to gate codes and pets.

How to use it

  1. 1Fill in the header. Enter the customer, address, date and any other details at the top of the form.
  2. 2Work through the items. Go through each item with the customer and check it off once you've confirmed it. Write the answers in the notes.
  3. 3Add notes. Write down anything the next tech needs to know, like a dog that gets out or a code for the side gate.
  4. 4Download or email the PDF. Download the completed form as a PDF to print or keep with the job, or email it to yourself to forward to the customer.

What a good new customer intake form covers

  • Contact details plus how and when the customer wants to be reached
  • Access information techs need on arrival: gate codes, parking, pets
  • Who owns the property and who approves work
  • The service requested and any history with the equipment
  • How the customer found you, so you know which marketing works

FAQ

New customer intake form questions

Have a question we didn't cover? Book a demo and ask a product specialist.

Should I take payment information on an intake form?

No. Don't write card numbers on paper or in a PDF. Collect payment through a secure payment system when the work is approved or done.

Why ask how the customer heard about you?

It tells you which ads, referrals and listings bring in customers, so you can spend your marketing budget where it works.

Where should I keep intake forms?

Store them where every tech can see them before a visit, and keep gate codes private. Field service software like Homeforce can store checklists on each job.
